To ensure your first visit is as smooth and beneficial as possible, here's a little guide on what to expect and how to prepare. Our goal is to make your check-in seamless so we can focus on giving your eyes the personalized attention they deserve.
Preparing for Your First Visit
To help us get to know you and your eyes better, we kindly ask that you arrive about 15 minutes prior to your scheduled appointment. This allows ample time to complete any necessary new patient forms.
What to Bring for Your Appointment:
For a comprehensive and efficient visit, please bring the following with you:
Your Photo ID and all current Insurance Card(s).
A complete list of all your health conditions and any allergies you may have.
A detailed list of all medications and supplements you are currently taking (including any eye drops).
Any questions or concerns you have about your eyes – we encourage you to write them down!
All glasses and contact lenses you currently wear. Please wear your glasses to the appointment, as we will use them for initial vision testing. Don't forget a pair of sunglasses for comfort after dilation, should it be necessary.
We also want to make your visit as convenient as possible, which is why we offer parking validation.
